Efficacy and Safety of Dalbavancin Monotherapy as Salvage Treatment for Bone and Joint Infection

Bone and joint infection (BJI) treatment is challenging with significant morbidity mortality. Dalbavancin a semi-synthetic lipoglycopeptide analogue of the teicoplanin class that exhibits bactericidal activity long half-life. The use dalbavancin may be an option in cases gram-positive BJI. From November 2017 to April 2019, was used monotherapy as salvage for BJI, follows: 1500 mg, 1st (D1) 8th (D8) days, repeated if needed. follow-up period 6 months osteomyelitis 1 year prosthetic infections (PJIs). demographics 16 patients showed 75% were men (n=12), mean age 77.8 years [64-90]. BJI characteristics: 5 vertebral osteomyelitis; 12 lower limb {8 infections, among which PJIs (4 knees, 2 hips) 4 foot osteomyelitis)}; shoulder PJI. debridement, antibiotics, irrigation, implant retention (DAIR) procedure performed 83.4% (5/6) cases. Monobacterial biopsy obtained (n=12) majority staphylococcus (15/25) susceptible micro-organismes): 14 Staphylococcus aureus (10/14 methicillin susceptible). Twelve received doses dalbavancin. duration antibiotherapy 18.3 days [0-49]. clinical success rate at end no major side effects This report highlights potential role efficiency treating fragile who require long-term antimicrobial therapy excellent tolerance profiles.
